To Report an Occupational Injury/Illness:

(Effective July 15, 2011 - ammended 9/2/11)

 

1) Make immediate personal contact (in person or phone) with a home base supervisor within 24-hours of the arrival of ID on which the injury occurred.

 

 - Contact needs to be made with your home base supervisor. If at an international location, see your FAOM for SOS/MedAire numbers.

 

2) When filing a report, have as much information available as possible (i.e. aircraft number, equipment type, location, witnesses, etc.)

 

3) If unable to continue working your current ID, or if unable to work your next ID, call OPBSK and advise the absence is for a work-related injury.
